  2. Narcolepsy: It manages excessive daytime sleepiness and sudden sleep attacks.

It works by increasing the levels of dopamine and norepinephrine in the brain, which are important for attention and behavior regulation.


Dosage
The dosage of methylphenidate depends on the patient's age, condition, and response to treatment.

  • Immediate-Release Tablets:

    • Adults and children 6+ years (ADHD): Start with 5 mg 2–3 times daily before meals; may increase by 5–10 mg weekly. Maximum dose: 60 mg/day.
  • Extended-Release Forms (e.g., Concerta, Ritalin LA):

    • Adults and children 6+ years: Typically, 18–36 mg once daily in the morning; adjust as needed. Maximum dose: 72 mg/day.
  • For Narcolepsy:

    • Similar to ADHD dosing but tailored by a healthcare provider.

Note: Follow the prescribed schedule strictly, and avoid taking the medication late in the day to prevent insomnia.


Side Effects
Common side effects:

  • Nervousness or anxiety.
  • Insomnia or sleep disturbances.
  • Headache or dizziness.
  • Loss of appetite or weight loss.
  • Nausea or stomach pain.

Serious side effects (seek medical attention):

  • Rapid heart rate, chest pain, or difficulty breathing.
  • Severe mood changes (agitation, aggression, depression).
  • Circulation problems (numbness or color changes in fingers/toes).
  • Symptoms of an allergic reaction (rash, swelling, trouble breathing).

Precautions

  1. Heart Conditions: Methylphenidate can increase blood pressure and heart rate. Avoid use if you have serious heart issues or arrhythmias.
  2. Psychiatric Disorders: It may exacerbate anxiety, agitation, or existing mental health issues (e.g., bipolar disorder, psychosis).
  3. Growth Suppression: Long-term use in children may affect growth; regular monitoring is essential.
  4. Drug Interactions:
    • Avoid combining with MAO inhibitors (risk of severe side effects).
    • Inform your doctor about antidepressants, antipsychotics, or other stimulants you are taking.
  5. Pregnancy/Breastfeeding: Use only if clearly needed. Discuss risks with your doctor.
